As for me, my guess is that it's a classic both-and... providers and insurers contribute to the cost, as well as the legal system, the culture, the psychology, etc; where the biggest 'culprit' is unconsciousness and an incentive landscape that is locked into place.... This article argues that insurance companies are only minor contributors to the US healthcare problem - the main culprits are providers (hospitals, doctors, pharmaceutical companies, etc.) who charge excessive prices while maintaining a friendly facade and letting insurance...
